TOPIC: Allow 2nd form to collect additional custom fields

I would like to allow to link in another 2nd custom form after the first form is confirmed, to collect additional information based on answers to the first form.

For example, if the user say they are a certified (i.e., PMP) on the question on the first form, the second form would ask for their PMI number.

Provie the ability to send an additional email to user that registers based on how they answerd a question on the form, in a specific way, i.e.,
if they say Yes they are a PMP, by checking the PMP checkbox, the RSVP system would send a special additional formatted PMP email to the user registering.
