TOPIC: A real waiver field

It would be great to have a configurable waiver/terms field that might look a little nicer than a checkbox with a really long label.

One of my users suggested:
Yes/No rather than check box for agreement to terms would be preferable
Text explaining the double arrows for the [Captcha] would have been helpful
