DT Register does not currently have full Joomfish support. We will be adding it very soon... hopefully in the next release. Currently, if you translate the language files, they WILL be used appropriately by Joomfish, but you will not get translation of database info like custom fields, category names, etc. So it's close, but not 100% yet.
