TOPIC: [SOLVED] Form Not working

I cannot get the first form to progress forward. the continue button is not working. Is there anyone who could help here. Please I have to launch this next week.

For the sake of others reading...

The problem is that this user had ECheck enabled along with PayPal. However, they don't have authorize.net so they can't use the echeck option. Also, to proceed on the payment type selection page, you have to first click a radio button before the next button will work.
